September 2025 Update:

At this time, EPS is in network and will accept the following insurance plans:

- Aetna: Allied Benefits System, Gravie

- Cigna Evernorth

- Medicare: Perennial Advantage

- UMR (United Medical Resources): third-party administrator for certain UHC plans including Quantum

- UHC (United Health Care): Doctor's Choice, Optum, Oracle, Surest

EPS will also accept non-network rates for Tricare/Triwest beneficiaries under the following conditions:

- Tricare/Triwest Select: no referral required, but an authorization will be completed by EPS to request evaluation test units

- Tricare/Triwest Prime: 8/21/25 - difficulties with acquiring PCP referrals have resulted in EPS not being able to schedule Prime covered members unless they share the required referral

​***Always check with your insurance plan to see if we are in network with a particular subplan. For example, we are not in-network with Medicaid, or any Medicaid Advantage programs under UHC, including Rocky Mountain Health Plans (HMHP).

It is important to note that individuals covered by Medicaid CANNOT request services under "Self-Pay". State Medicaid protections prohibit providers from charging Medicaid patients any out-of-pocket costs, including the entirety of an evaluation. As a result, no practice may charge a Medicaid patient any Self-Pay fees.

Self-Pay

If you would like an evaluation from EPS, but have an insurance outside of those we are contracted with, we do offer self-pay options. If you are considered a Self-pay client, you will be expected to pay fees in full at the time of service in accordance with our Billing Policy. Payment is accepted via cash, check, or credit card.

For all new clients, EPS requires that new client paperwork be completed so that insurance verification and any required prior-authorization approvals be processed as quickly as possible to proceed with scheduling the evaluation appointments.
